Home Visits 

The St Helens and Whiston Hospital Rheumatology team offers a very limited home visit service. This service is obviously very resource-intensive which is provided by therapy and nursing service and we are therefore only able to offer it to a small number of patients for whom it is the most appropriate service. It will be discussed with patients for whom it is thought to be appropriate when they attend clinic or are discharged from Hulme Ward. 

What happens:
An appointment will be sent to inform you of the time and date of visit.
You will be contacted on the day of the appointment to ensure you are available
A nurse practitioner and therapist will visit you at home
The team will introduce themselves
They will assess your rheumatological condition including drug medication and monitoring, your functional ability including mobility, how you manage around your home, your care package and family needs
They will agree a management plan with the patient and the family
They will write to your GP (You may, if you wish, request a copy of this letter at the time of the visit)
They will inform you of the approximate time of your next visit (usually every 6 months)
You will be given a contact number to the rheumatology Nurse Specialist advice line 01744 646457 should you have any problems in between visits.
