Sustainability and Transformation Plan

The NHS is facing challenging times. To meet these challenges, the NHS and its partners need to work together to ensure that heath and care services are built around the needs of local people.

The Trust is working with its partners across Cheshire and Merseyside to look at how services are currently delivered across the area, and ways to make them more sustainable in the future.  The aim is to help people to live longer and healthier lives, whilst at the same time, providing the best possible services to those who require them.

The Cheshire and Merseyside Sustainability and Transformation Programme (STP) has developed draft proposals for how to move forward and develop plans for the future. These plans have been submitted to NHS England, and over the coming months engagement with the local community and key stakeholders will help to shape these proposals further.
